c. A note telling me what you’d like to have done.
d. Your name / address / email / phone
e. Payment in the amount of $10.00 (non-refundable) for return shipping within the U.S. (For shipping outside the US, please contact me.)

When I receive your Folstaf® I will examine it and contact you to discuss what needs to be done, tell you what is/is not covered under warranty, and what costs, if any, are involved.
With your approval and payment, I will repair your Folstaf.
I will return your staff to you. Average turnaround time is three weeks.
